Permission Slip
The scout must carry the “Activity Participation Authorization” with them at ALL TIMES during the activity. It must be signed by the parent(s) or legal guardian before the scout leaves on the activity.
Birth Certificate/Passport
Activities that take place in Canada, require the scout have:
- a copy of their birth certificate AND a photo ID, or,
- a passport
These documents must available for inspection when we cross from the U.S into Canada, and when we return from Canada to the U.S.
Medical Forms
The old Class 1, 2, and 3 medical forms have been replaced by a new single form. A “Frequently Asked Questions” (FAQ) concerning this new form can be found here.
IMPORTANT NOTE – In the past, many of our scouts used Sports Physical forms in place of Class 2 forms. With the adoption of the new form, Sports Physical forms are NO LONGER ACCEPTED.
